Registration Form

To register at Immaculate Heart of Mary parish, you have three options:

1.  Come to the parish office, fill out a registration form,  and receive a packet of information on the parish.

2.  Click here and print out the registration form and return or mail to the parish office at 7820 Beechmont Avenue, Cincinnati, OH 45255.

3.  New Parishioner Registration is conducted after all Masses on the first and third weekends of each month.
